Our referral and admissions process is designed to be respectful, responsive, and centered around the participant’s service needs and ISP goals.
Our process includes:
- Initial inquiry or referral
- Review of participant needs and current supports
- Discussion of ISP goals and available services
- Coordination with family and support team
- Intake and onboarding
- Service implementation and ongoing communication
We welcome referrals for:
- In-Home and Community Support
- Companion Services
- Respite Services
- Community Participation Support
- Transportation
